Stretch-n-Grow is a physical fitness and exercise program. Your child will be actively exercising through cardiovascular and callisthenic exercises for approximately 30 minutes once a week. Exercise consists of age appropriate warm-up, floor exercises, and a cool down. Please list and describe below any injuries, allergies, or health conditions, which might limit your child’s participation or check the “none known” box if none. Thank you.

Parent policy:

Our tuition accrues monthly and is due on the first class of each month for Stretch-n-Grow and Dance (camps excluded).

	Payment must be made regardless of the child’s attendance while enrolled.
	Our tuition fees are based on four-week months. 5th weeks are used for make-up classes for Stretch-n-Grow and dance classes only.

	Payments not received by the 10th are subject to a $5 late fee.

	There is a $30 fee on all returned checks.
	Your child will not be added to the roll until payment is received. Please allow 2-3 days for processing.

To withdraw your child, you must call or e-mail our office. If you fail to notify us directly, billing will continue to accrue for which you will be responsible.

Your invoice will be e-mailed to you once Stretch-n-Grow has received your child's enrollment form.

In consideration for your child being in Stretch-n-Grow, you hereby release SNG from any claims, demands and causes of action arising from your child’s participation in the program
